Inpatient Functional Screening Tools

We are reevaluating our current functional assessment we use upon inpatient

admission. Currently we have several questions that if answered positively,

automatically requests a screen from PT/OT/Speech. The problem is that our tool

is actually too sensitive and we are getting many people referred for screens

that don't need therapies.

Would anyone be willing to share their functional assessments/therapy screening

process for inpatient admissions? Thanks.
