We typically terminate the PT's when they leave our facility from any individual contracts. Most of our contracts are now group including Medicare. For Medicare, you may want to reassign your individual number to a new group Medicare number for your business. (Medicare often assigns a separate 855r number to reassign your individual number to the particular PT practice.) Many of the other private insurance providers will simply have you under the practice's group provider number, so you won't have an individual number to take for those insurance companies. Doesn't matter if you have already been credentialed by your present employer. Guest guest Posted June 12, 2011 Report Share Posted June 12, 2011 Joe, As someone who just opened a new practice I can say absolutely yes you will need to be recontracted and recredentialed with most providers. This can take time so start now. I would recommend calling and asking each what steps to take but most will at the very least need a Provider Change form and many will require new contracts. Most have applications online. If you are starting your own practice you may be better off just getting group contracts for your practice so that you don't have do contract each provider and do a reassignment of their benefits. I have found no way to purchase a PT database with email addresses, etc., but would love to be able to purchase such a database.
