Does anyone have a confidentiality/HIPPA agreement for volunteer/student

observers that you would be willing to share. Is there anything needed from

volunteer/observers as far as vaccine history, TB test, etc. that I should be

aware of. I am a better P.T. than administrator. Thank you in advance. You can
