I don't have problem with reassessing patients, because most of us do this every visit as we should. The difficulty is with multiple disciplines, say a CVA with PT, OT, and ST referred. With 3 disciplines, we must " reassess " the patient close to the 13th visit, and close to the 19th visit (no later than for either). Well, I have had patients where I reassess on the 13th visit, and my next visit there are due for another reassessment (because OT and ST have been in since my last visit). Theres nothing to reassess, one visit had occured. 3 disciplines should only have to reassess prior to the 20th visit. The problem compounds if you also use a PTA for visits. I could handle the single discipline but when you combine. whew. I just wonder if I am hearing the info correctly.
