Kieth, it tends to be up to your licencing board requirements. In Minnesota, the

PTA writes the note and signs and documents that they reviewed plan of care with

the PT and the PT just signs the note.

Can a PTA document progress notes....

Can a PTA document progress notes if the PT interacts with the patient and signs

the note? We use EMR and I'm trying to figure our how to document progress notes

for patients on PTA schedules. Can the PTA document my assessment in a daily

note and I sign the PTA's daily note if it contains adequate " progress note "

information. or do I have to write a separate note?
