Below is a checklist of a shoulder eval. Are their any areas that I have

missed?

Shoulder Checksheet

_____History

Do your symptoms change with any movements of the neck? (cervical spine

involved)

Does your arm ever slip out or feel unstable? (instability)

Does your pain change with overhead activities? (impingement syndrome)

Does your arm not move because of pain or is it unable to move? (r/o RTC

Tear)

Does your arm ever feel heavy after performing activities? (Vascular

compromise)

_____ROM (C-Spine screen if applicable)

AROM/PROM and overpressure - Flexion/Extension, Abduction/Adduction,

Internal and External Rotation (neutral and @ 90 Degrees Abduction), Horizontal

Abduction/Adduction, Hand behind back, Hand behind head.

Joint Play: Glenohumeral joint inferior glide, anterior glide, posterior

glide

AC joint anterior/posterior glide, SC posterior glide

_____Muscle Testing: Shoulder flexion/extension, abduction/adduction,

internal and external rotation, lower trapezius, tested at various ROM’s,

_____Neurological Testing:

DTR’s – Biceps, Triceps, Brachioradialis

Dermatome Assessment C4-T1

Myotome Assessment C4-T1

Adverse Neural Tension Testing – Median/Ulnar/Radial nerve bias

_____Observation/inspection:

Postural Assessment – Cervical spine position, scapular position

(elevated/depressed, protracted/retracted, winging/tipping, upward/downward

rotation),

Thoracic spine position, Scoliosis, rib cage position

Muscle wasting/atrophy

_____Palpation: Pecs, Lats, Subscapularis, Supra/infraspinatus, Teres

major/minor, upper traps, lower traps, serratus anterior, anterior/posterior

ribs,

omohyoid

_____Special tests: Hawkins-Kennedy Impingement, Painful arc sign (between

60-120 in scapular plane), Speed’s test, Sulcus sign, Apprehension test,

cervical quadrant to r/o c-spine pathology.

PT Diagnosis – rotator cuff tear, Tendonitis, Impingement syndrome,

instability, cervical radiculopathy, s/p surgery etc...
