You need to fill out 855i and 855r forms for medicare provider enrollment.  You

can go to the medicare website and print these and then send them in to be

credentialled and given a medicare provider number.

     I am joining the theraphysics network to able to become a provider for

several insurance carriers...They want me to be medicare certified... what does

that entail???  I am in a rural area of Colorado...They are getting back to me
