Our outpatient clinics are incorporated as Rehab facilities, not private practices that may have affected our decision also - we may not have been able to participate - I can't remember... Fall risk(100F/1101F), patient co-development of treatment(G8437), and pain assessment(G8440) are the codes that we use. They have to be done and billed with the evaluation to be eligible for the bonus to be paid next July 2009. You have to have these included in 80% of your medicare evaluations to receive a bonus. You do not use the GP modifier with these codes or they will be ineligible.
