There are none. Medicare only requires long term goals.

If you look on the 700 Medicare Form...you will see STG and LTG listed. I am not

sure if that means you must use STG but it is listed on the form.

Medicare addressed the use of the 700/701 forms in 2003:

yes the 702) were never required, (CMS stopped short on that), and this

transmittal clarified that the reference to the forms were being placed in

never-never land with respect to the policy manuals. You can still download

the CMS 700 and 701 forms at the CMS website on the forms page.

As for the non-requirement of short term goals - that was originally

addressed in 2005 via Transmittal 36 to the Medicare Benefits Policy Manual

(red italic ink) section 220.1.2 B.

incorporated into the Manual, and only long term goals have been a

requirement of the Plan of Care since 2005. Rather than view that short

Policy Manual with respect to their optional use, and in what instances they

might be used in your practice.

The long and short of it (no pun intended) is that CMS substantially updated

and (continues to clarify) documentation requirements several years ago.

Providers still utilizing the old paper & pencil 700/701 forms with the old

documentation requirements are likely to experience trouble under any level

of review either by the MAC, the CERT or the ZPIC or the RA (formally the

RAC).

CMs has left the matter of from and additional content to the clinician,

the focus for CMS in on the " at a minimum " requirements. Many clinics and

EMRs develop a form that looks somewhat similar to the old CMS 700/701, but

ensure that all required and " suggested " content is included. Some clinics

create a " serial " plan of care form that compares " progress " from one

certification and/or progress note to the former (to demonstrate comparative

analysis objectively and subjectively) in a series and use this serial

progress note also as an updated plan of care if all the requirements are

met.

A great source of information is your MAC's local coverage determination and

their educational events on outpatient therapy, and any Q & A format that

may be on their websites. One year at the Florida annual education event

for therapy the MR personnel passed out (redacted) charts on 6 cases, and we

did round robin reviews in groups. Then the MR folks told us what the

result was on each case during medical review. The information was

invaluable for the 50 providers that attended.

I second what said and will go one step further. I think providers should

shred all 700 and 701 forms as they do not allow sufficient space for good

documentation, are outdated, and are confusing to providers. Why not just use

your initial evaluation as your plan of care that you send to the physician for

his/her signature? All the required elements for the POC are already in your

initial eval, why do extra work?
