Most of the true " costs " of an EMR are typically in time: administrative time and, especially, physical therapists' productive time. That said, PracticeFusion is an ad-supported, physician-oriented EMR that charges you ZERO money in up-front or ongoing or license fees. They make their money on ads. PracticeFusion will, in turn, recommend an array of private support technicians certified by PracticeFusion. These people will work for you, not PracticeFusion. They will help you with training, building specialized templates, inserting alerts and prompts, running reports and all of the typical stuff that your current EMR charges you for. Which is better? I'm not sure. I can say that my local, 700-bed private hospital is using PracticeFusion and that my physician friends have not more complaints about this EMR than they have about any other EMR. They notice the ads but say they are not influenced or distracted by the ads. With any of the free EMRs, I like the fact that we as clinicians don't have to make a big, up-front dollar commitment, as if an EMR is an " investment " that will someday pay off. It won't. All your up-front dollars are wasted. Soon, the EMR will be virtualized by state-regulated, locally organized ventures called the Regional Health Information Organization (RHIO). All data will flow upstream. If your data doesn't fit the RHIO then your EMR is just a hobby - something you spend time and money on but doesn't pay you anything. We are on a quest as well and find a good billing and not so good clinical software or visa versa.
