My organization does have a form and a process for these students. Feel free to

Subject: Student observations

I was recently contacted by a college student requesting to come to my

clinic to complete a portion of

the 50 hours of PT observation required for her application to the PT

program at South Alabama.

All the student will be doing is observing and not in any way assisting with

treatment.

Does anyone have a form that students sign that observe in their clinic?

I would think that some type of form that advises them of the

confidentiality and HIPPA compliance

would be necessary?
