Guest guest
Hi, Mark - I'm interested in whether you believe that you are " losing money " because of: 1) Missed opportunities because of limited capacity? or, 2) Spending more than you take in? Which situation best describes your practice? Dr. Dick Hillyer Dr. Hillyer, PT, DPT, MBA, MSM Hillyer Consulting 700 El Dorado Pkwy W. Proposed Solution: Reduce visits from 60 minutes to 30 minutes. Problem: My therapist is afraid of the case load and states this is not good care. I have heard most private practices do 30 minute appointments. I do not have any data from hospital based therapist. What I need: Is it appropriate to expect my Speech therapist to do 30 minute appointments? Thanks, Mark

Guest guest
The answer depends on the make up of your pediatric caseload. In general your therapist is correct in providing hour treatment sessions. It is very difficult to make any progress with speech and language kids in a half hour session. If you would like to disscuss the types of patients that would be most appropriate for half hour sessions you can call me or email me offline. A. Guest guest
Mark I cannot answer what is or isn't appropriate for speech therapy treatments. But I would ask if you have thought about not participating with insurance companies on the speech therapy treatments? Instead, bill the patients and let them file their own insurance. Guest guest
Mark, We treat pediatric pts for 30 min unless the therapist deems more is necessary. Most of our kiddos see more than one discipline as well so 30min with each therapist is enough. We do have a high no-show/cancel rate in peds greater than our adults. However, the therapists (not front office) are encouraged to call the family 10-15min after their missed appt time. This has helped and keeps the families accountable. Hope this helps! Guest guest
We also have a large wait list and a few years ago it mushroomed due to some clinics closing, some not taking Medicaid, etc. We did a survey of other clinics, both private and hospital based to determine what other clinics were doing regarding the same issues of huge wait lists and the 60 minute, 45 minute and 30 minute treatment sessions. What we found was that most clinics offered the 30 minutes for " non-complex " patients and then the majority of appointments were 45 minutes. We moved to this type of treatment and we were able to see more patients and found that the quality of care did not diminish. We also added a lot to the home programs, and this has helped. We are now working on our cancellation policy, and if it really isn't working after 2 - 3 misses, and the therapists discretion, we take them off the schedule. It is a challenge. Melinda Melinda J.
