Would you be comfortable sharing with us why, when, and how your office uses this document? I as a billing office worker I see the document as a way to be compliant with the ins. cos. we have contracts with so it does not appear our fee schedule changes from patient to patient. Very simply it can't be a blanket policy, must be used on a case by case basis, and must initially obligate the patient to the full amount owed. Happy to share more details. Thank you!
