Guest guest Posted December 29, 2011 Report Share Posted December 29, 2011 Tom If the PT is the only owner of the corporate entity, and uses that entity to bill Medicare, the application process is simplified in one application, with no need to complete the 855b (Group) or the 855r (reassign benefits). (or the PECOS equivalent) .. Complete the proper form (855i) in paper format so that all the information is readily available and accurate when creating an enrollment or revalidation in PECOS .. All names should correspond in exact form to names that the Federal Government has: o For individuals - corresponding to records on file with the Social Security Administration o For businesses - corresponding to records on file with the Internal Revenue Service and the associated TIN as applicable .. The revalidation letter will clearly indicate the NPI and PTAN numbers that are being revalidated - to that end a copy of the original 855 application will assist in this process as to the correct form(s) to utilize o Most, if not all, contractors have published " sample " revalidation letters for reference .. For PTs that are " solo " owners of their company, and will bill Medicare through that entity the application process incorporates the 855i, 855b and 855r as one application. The directions are clearly written on the 855i form. o In other words if you are the single owner of your entity, and will bill Medicare through that entity, personal information is completed up to Section 4 of the 855i, and information regarding your entity is used to complete the remainder of the application (from Section 4 to the end) o There is no need to complete the 855r (to reassign benefits) for the owner of the entity as described in the above. If the Idaho PT fits the above scenario, then the Managing employee section must be filled out. The key is to check the top of the revalidation letter to determine who/what is being requested to revalidate. This is why is will be helpful to have a copy of the original application if available.
