Concurrent vs group charges in IRF

The Manager of our IRF asked me to post the question below to the group for

discussion. First Coast is the FI in Florida.

Inpatient Rehab Facilities are not paid by the CPT, but by the visit. CMS has

made it clear that therapy for IRFs are to be intensive and primarily

one-on-one. They have allowed group therapy and historically have not ruled out

the possibility of concurrent therapy.

For IRFs, are concurrent therapy sessions (where more than one patient is seen

by one therapist) ruled out completely at this time? How does First Coast

interpret billing for more than one patient in an IRF? Is it possible to have

concurrent therapy?
