This topic comes up monthly. Thought I & #39;d share something that I found.

Please reply if you find the claims to be reasonable. This is packed with info.

Note, the PT clinics were owned by " orthopedic groups " --presumably MDs--for what

billing rules commonly known as the " one-on-one rule " and " 8-minute roll-up

rule " to non-Medicare patients.

I read the information and was surprised to see that the author thought the

" one on one " rule was merely for Medicare. As I understand it, although

Medicare uses that requirement in areas such as Therapeutic Exercise that is

the CPT copyrighted definition of what is allowed to bill for codes such as

Therapeutic Exercise (and some other codes, i.e. Neuromuscular Re-education)

and If you bill using CPT codes you can not change the copyrighted

definitions of the code and therefore the one on one rule stands for all

patients that are billed using CPT codes that require it. Am I missing

something?

The author suggests, among his reasons for " low " productivity, that " The

provider is unnecessarily applying the relatively restrictive Medicare billing

rules commonly known as the 'one-on-one rule' and '8-minute roll-up rule' to

non-Medicare patients. "

He expects one sort of billing protocol for Medicare, and another for private

insurances. It seems to me that that ought to ring alarm bells. If you are

billing CPT codes, then you are representing that you are following the

definition of those codes, which are clearly described according to time and

attendance (special contractual arrangements aside).

Damon Whitfield recently asked a related question under the thread " Billing

times and units for varying insurances " and did not get any responses. Maybe

this is a good time to discuss this?

Is it my imagination but does that seem like a lot of procedures per

hour/day. (4 timed procedures+1 modality per visit @ 9 visits per day (45

units billed per day if I read this correctly). Article was written by Cary

B Edgar who is a lawyer and former CEO for an unknown outpatient rehab

company.

Also, I see they are establishing benchmarks for physicians in a group

practice also which I could partially see thru search terms. I could not

tell if they were as rigorous as the PT ones.

PT must be a business with expectations but I am surprised that they would

actually want these standards published for any public review.

I will admit they had some good suggestions for problems with the practice.

Missed visits, understaffed, etc. all basic and so easy to overlook. If

you add the correct staff with the right conditions . they will come.
