Trouble with online Medicare revalidation

Hi everyone,

I have a question from an Idaho PT about Medicare revalidation. She had a

rejection of her online application due to " not listing the managing

employee " . She said that she could not find the location to enter this

information on the online format and when she called to get help she was

told to " read the instructions " . I have offered to help but I am not yet

familiar with the online revalidation format so I am asking your help to

see if you can let me know what she should do. Thanks
