Our office has been very successful in doing these exact measures for all of

our patients for several years, fall codes for anyone 55 yrs and older; and

pain/medications for anyone 18 and over; (and yes, getting paid the

incentive payments by MC). I have a cheat sheet that our therapists use,

and would be happy to fax it to you if you would like. Our patient intake

form has most info for the patients to complete (not pain level), and our

therapists document the pain, fall info, and medications in the IE's as

indicated. All of the codes for the measures were added to our billing

software (PTOS); and the therapists do the coding at the time of the

evaluations. We have added many fall/balance measures, to support the use

of some of the codes.

These measures are only done on IE's (billed 97001 and 97003) there may be

others, but those are the ones used in our office.

I did research in detail a couple of years ago, and went to a MC course in

Jax, FL for PQRI measures, but I don't have the MC website location for

these measures off hand anymore. I am sure it is still well documented on

MC website, www.fcso.com; so just to CYA, I would say that would be my main

resource.

I am a little disappointed others don't respond to your questions, but

realize it is hard to put clinic info out there to be scrutinized, again

CYA.

Another thing, we do NOT use a clearing house, all claims are entered in

house, submitted electronically. With this, we enter Zero as the charge

amount for the above codes and remove units from the claim line, but all

other info like dx, is kept on the claim line. I have heard for

clearinghouse billing you must enter a billed amount (like $1.00), so as not

to generate an auto-delete of the claim line. I think clearinghouse issues

are why others have not done these measures.
