RE: Inpatient skilled nursing and rehab admission process

We do a " Pre screen " that the DON or ADON and myself review for rehab

potential and participation and medical concerns or questions that arise

from the paper review. Then the Social Services Director and sometimes a

nurse go to physically look at the potential admission.

We are looking at restructuring our admission process for our post acute

care areas. I am wondering, for those of you at this level of care, are your

admission screens done with an on site in person assessment of the patient

or are they done remotely with review done via clinical records?

For our inpatient rehab admissions, we have an RN Preadmission Nurse asses the

patient on site. The only time we do a records review only would be if the

patient was referred from out of state.
