I cast for them with a subtalar position and then send off to a lab which has

worked out great. You can buy equipment and grind down your own orthotics;

however this becomes much more labor and cost intensive upfront.

Hi Dave,

I have done both in clinic fabrication and in clinic casting or molds and

the in clinic casting/molding was much less time consuming and much more

profitable but you have to make sure to work with a company with great

customer service and fast turnaround for updates or repairs. Be happy to

make a recommendation off the list if you want. My clinic has been working

with the same company since 2000.

Dave,

Yes, I primarily do one of 2 things....I use an outside lab for custom

orthotics (30-40% of the orthotics we do) and Vasyli off-shelf,

semi-custom orthotics for 60-70% of the patients. The Vasyli devices

(available through Sammons-Preston/ Medical) have worked very

well, are easily put together (there is a variety of modular add-ons for

rearfoot and forefoot posting, heel lifts, etc.) and easily molded to

the patients' feet with the use of a heat gun. Feel free to e-mail me

directly or give me a call if you like and we can talk more. I do not

get into the grinding and fabrication of truly in-house custom devices.

That being said, with the combination mentioned above we have had

terrific success.
