Our policy in outpatient was that the PT who completed the evaluation was

the PT of record and if that PT was unable to continue with that patient,

then there would have to be some documentation of the transfer of care to

another PT. Same happened with vacation coverage where there would be some

record of the transfer of care to the covering therapist.

This is difficult for inpatient care but there still should be some policy

to offer guidance. It is also difficult in facilities that do not encourage

the same PT or PTA to stay with a patient throughout their care.

Check with your state licensing authority to see if they have defined PT of

Record. If not, a policy such as suggested is reasonable.
