Guest guest Posted March 20, 2012 Report Share Posted March 20, 2012 Our policy in outpatient was that the PT who completed the evaluation was the PT of record and if that PT was unable to continue with that patient, then there would have to be some documentation of the transfer of care to another PT. Same happened with vacation coverage where there would be some record of the transfer of care to the covering therapist. This is difficult for inpatient care but there still should be some policy to offer guidance.
