Guest guest Posted March 21, 2012 Report Share Posted March 21, 2012 I was just cruising thru my state newsletter yesterday and noticed a few therapists who got a disciplinary action for not having 30 re-evals or dc summary. I started thinking that I was maybe hanging my butt on the line since I do some weekend evals for a local HH agency and then turn the patient over to their regular staff of PT /PTA team. What kind of statement should I write to have coverage that the case is actually picked up by the regular PT for coverage. I know the PTA well and she is meticulous about this but just wondered if others make a statement on the eval about releasing care to other PT. The PTA calls me on Monday if there are questions.
