Guest guest Posted April 13, 2012 Report Share Posted April 13, 2012 Would like input from those in inpatient rehab and how they do discharge summaries for therapy disciplines? We are in Texas if it helps with requirements. Texas board states for physical therapists: " Discharge Summary. The PT must provide final documentation for discharge of a patient, including patient response to treatment at the time of discharge and any necessary follow-up plan. A PTA may participate in the discharge summary by providing subjective and objective patient information to the supervising physical therapist. " 1. Are they required at all for IRFs? 2. If so, anyone have any other links to specific requirements? 3. If a PTA is the last to see a patient, could they write the final note- list functional levels, state patient has met goals, and is being discharged and have PT co-sign that note?
