Acute care therapy orders

Are there any hospital departments out there operating on an " eval and treat "

order only or are you getting a clarification order for ICD-9 code, frequency,

and plan of care just like outpatient orders? Thanks for your input.

We have an EHR and have built our orders of eval and treat to include a reason

for referral that is selected from a pick list. As in the outpatient world, we

have the physician cosign our treatment plan including frequency and duration.
