Rick Gawenda question: referenced definition of fee slip or billing slip

What is the definition of fee or billing slip? Is it the same thing as an

individual 15-minute CPT code; the CPT codes charged per client visit OR the

fee slip/billing slip sent by (in this case a PT office) that lists the

clients and CPT codes for each visit?

One local billing service charges per 'fee slip " which appears to be not per

client, but per code which is ridiculous. I want to make sure I understand

industry definitions of billing terminology.
