only if you are a C.O.R.F, these facilities require a Medical Director becuase

they have direct access to pt's w/o a referral. I am in florida so not sure if

your state is the same.

paperwork for The Flagler Institute for Rehabilitation. The email contains

documents that MUST be completed including a word document and a pdf for the

intake information and optimal survey form. Optional documents that are also

attached include a pdf of our brochure, staff photo, and directions to the

facility. For ease of intake, please complete prior to your first visit. Please

arrive 30 minutes prior to your first visit to clarify information and possibly

complete additional insurance required paperwork. You will also receive a second

email confirming receipt of our newsletter. If you have any questions regarding

Financial responsibility is always important to clarify as we do NOT want any

misunderstandings. The policy of The Flagler Institute is to collect at the time

of your visit any Co-Pay, Co-Insurance, and/or Portion of the Deductible on the

date of service. We accept Visa, and Mastercard, HSA Visa Cards, Check and Cash.

We look forward to providing you the optimal health care available.

A Rehab Agency does not require a Medical Director by the Conditions of

Participation, nor have the Interpretive Guidelines suggested that as a guidance

for surveyors. The Interpretive Guideline for Condition of Participation

485.709(d) requires that the “facility should have written patient care

policies, based on accepted standards of practice for all services provided,

that govern the outpatient physical therapy and/or speech pathology services and

related services that are provided. Patient care policies are established by the

professional staff of the clinic or rehabilitation agency and, where

appropriate, outside professionals who function as a patient care policy

committee. The professional staff preparing the patient care policies should

include at least one physician and at least one qualified physical therapist (if

physical therapy services are provided) and at least one qualified speech

pathologist (if speech pathology services are provided). “ Rehab agencies can

fulfill this requirement by having a physician participate annually in the

review and updating of policies, which does not require a medical directorship.

Additionally the Rehab Agency requirement to have a physician on call for

emergencies was eliminated several years ago, although the interpretive

guidelines have not been updated to reflect that (and other )changes in the CoP.

A CORF requires physician services as a core component by the Condition of

Participation 485.58(a), and most CORFs as part of fulfilling this requirement

(in part) by establishing a “medical director†relationship with the

physician who is fulfilling the CORF core physician service requirement.
