Hi Pamela,

You have to do a new evaluation and POC anytime a resident is admitted to

the hospital or is out of the building for more than 24 hours unless on a

leave of absence. The eval is still focused on the initial admit Dx, not the

new one from the hospital. If you are ever unsure of whether a new eval/POC

is required, find out if the MDS coordinator did a D/C MDS with anticipated

return, if they did then you must do a new eval & POC.

I am at a SNF in Alabama. I have a question about a Resident who is on part

A short term rehab s/p THA. She went out to the hospital and was admitted

for bladder stuff on Saturday. She was readmitted to the SNF on Sunday.

Question: Do we need to do a full eval even though what she went out for

was not related to the THA and furthermore has had no change in function.

Will a reeval suffice? Thank you for your assistance.
