Hello Sam,

The difference between Med A and B is that the certification requirement for

Part A is done by the facility for the total care the patient requires for

the stay. It is done on admission, then by day 14 and then every 30 days

after that. The therapy plan is not " certified " as it is in Part B but the

physician reviews and approved the plan for the duration of the plan. So if

the plan is for 6 weeks, that's how long the plan is approved for. If the

patient needs further care, that procedure is covered under the facilities

own P & P. Some will get a telephone order to continue for 1 or 2 more weeks

depending on the reason for not meeting the goals in that time frame, others

will perform an updated POC and submit for approval.

CMS has never had written guidelines for documentation per se for SNF Part

A.

Part B's certification is for the longest time frame identified by the

frequency & duration, but no more than 90 days. So if the plan is 3xw for 6

weeks, it is either 6 weeks or 18 visits before an updated POC needs to be

done for recertification.

I would refer to the RAI manaul for the Part A

cycle - this manaul walks you through how to fill out the MDS and all the

criteria/rules.
