You can start with the regulations and guidance section of the CMS.gov

website. This covers the conditions of participation. You then want to

contact the regional office for your area. The map link is provided below.

http://www.cms.gov/About-CMS/Agency-Information/RegionalOffices/RegionalMap.html

Once you determine your office this is how you contact them:

ect=/RegionalOffices/

You would be given a survey worksheet for the items that would be reviewed

prior to operational approval.

- I providing this response I am assuming that you are a PT in

private practice.

MSM has been awarded the National Site Visit Contractor (NSVC) contract by

CMS to conduct site visits under NEW enrollment requirement, including

revalidation, that became effective in March of 2011. Physical therapists

in private practice are in the " moderate " risk category, whereas

occupational therapists in private practice are in the " limited " risk

category. A physical therapy group practice will be subject to the

" moderate " risk category also.

PTPP are subject to additional screening criteria during either new

enrollment, or revalidation of either the individual therapist or the group

practice. The CMS announcement is here:

ProviderSupEnroll/NationalSiteVisitContractor.html>

https://www.cms.gov/Medicare/Provider-Enrollment-and-Certification/MedicareP

roviderSupEnroll/NationalSiteVisitContractor.html

Palmetto may have mislead you in saying the information is protected, as the

guidance on the NSVC program can be found in the Medicare Program Integrity

Manual (100-08) here:

83c15.pdf>

https://www.cms.gov/Regulations-and-Guidance/Guidance/Manuals/downloads/pim8

3c15.pdf References to the site visit criteria for physical therapist

can be found beginning on page 79, and again on page 230. The elements of

the site visit for physical therapists begins on page 234.

CMS is 1 year into the 4 year process of mandatory revalidations, they

started the process with those not enrolled in PECOS last year, and are

proceeding over the next three years to revalidate all providers and

suppliers. The revalidation notice letter will be received in the mail, and

CMS also posts the lists of those receiving letters to revalidate on their

website. If you miss the 60 day window to revalidate, your billing

privileges will be deactivated (not necessarily revoked).

As an FYI CORFs are in the moderate screening risk category, and newly

enrolling DME suppliers are in the high risk screening category. Both are

subject to surveys by the NSVC, which is MSM, or its subcontractors. (Health

Integrity LLC, and Computer Evidence Specialists LLC).

A final comment: This has nothing to do with Medicare Survey and

Certification for Rehab Agencies and CORFs.

Subject: Medicare Part B Clinic Survey

Does anyone know where to find a list of site requirements for Medicare Part

B oupatient clinics?

I called my local carrier (Palmetto) and requested a list of requirements,

but they said they didn't know and that they outsourced surveys to MSM

Security, which I also called. They told me the information was protected

and not available for release. So how do new clinics know what the

requirements are?
