Nutcase sounds more like something a nut would be encased in. Isn't it interesting how we have to " properized " (that is a Rennie term--grin) slang? Therefore, in my opinion, it should be enclosed in quotation marks. Surely the physician was quoting a lay person, not diagnosing! At least it was a printable colloquialism.
