Guest guest Posted August 1, 2012 Report Share Posted August 1, 2012 We are having difficulty organizing the team structure in our 40 bed acute rehab facility. We want to promote cohesiveness between therapy, nursing, case manager, and MD. Our goal is to improve team communication and enhance patient centered care. Structuring treatment delivery by specific interdiciplinary teams of clinicians seems to be the best way however because of the nature of the departments, their size, and their schedules we are having little success. Currently: we have in place 7 PT/OT teams assigned to room blocks based on nursing assignment to those rooms. This has had an inconsistent affect because of nurses schedules changing the consistency to assigned rooms and patient room changes secondary to medical and personal reasons.
