outpatient daily note

I am hoping to get some information on what makes a good daily note - one that

is compliant with the outpatient regulations and efficient for staff. We use an

electronic medical record (Epic) and are looking to streamline our processes.

Hi Norma,

It would help to know if you have reviewed the Part B Medicare Benefits

Policy or not since that has most of what you need to know as far as

documentation requirements for Medicare.
