I know I am a lurker on this list, but I do still reas some messages--and I read your last one and all of the replies that were posted to the list. Some suggestions, if you haven't heard them already: Your doctors (PCP and pain management) are committing serious malpractice by treating you this way. I don't know what the medical malpractice laws are in Pennsylvania, but I do strongly encourage you and your wife to find out. You could also file complaints with the state and federal medical associations, and the state medical licensing board. Make some noise. Maybe it will help. Another thing to consider, depending on where you live, is moving (closer) to a bigger city, such as Philadelphia, burg, or Pittsburgh. The bigger the city, the more options you will have.
