I also have had to keep a mood journal for depression before and there are some on the internet. http://www.ahealthyme.com/topic/journaltherapy Journaling is a positive therapy as it helps to identify what is bothering you and I was very surprised to find out some of the things. The American Pain Foundation has an on line pain journal you can enter online > Ask Bennie, how she writes her pain diary, she will know a good method.
