SimponiOne Patient Cost Support Program

Your eligible patients may qualify for instant savings on their

private insurance co-pay, deductible, and co-insurance medication

costs for SIMPONI™ (golimumab) through the SimponiOneSM Patient Cost

Support Program. If your eligible patients qualify and activate their

SimponiOneSM Patient Cost Support Program Instant Savings Card, their

out-of-pocket costs for the first six months of SIMPONI™ (golimumab)

are covered in full. For months seven through twelve, your eligible

patients pay $5 a month for every $1,005 portion of their

out-of-pocket drug expense. The SimponiOneSM Patient Cost Support

Program is subject to a $6,000 annual program benefit, 12 months after

activation, or 12 injections, whichever comes first.
