Usually the protocol to be put on a biologic is to have other meds not work that well alone, such as methotrexate, and plaquenil, or others. Not really fail, but usually, mtx will be increased to maximum dose for about 3 months. At this point the Dr. will submit to the insurance company his and his patients results to get approval for a biologic. Usually you have to take a TB test before starting one of them and then you can start. My rheumy had discussed with me several biologics, had given me all the pamphlets, samples of all to see how they are given, etc...I ended up choosing Enbrel and that is what he got approval for.
