For me, and I don't know if this is how it affects everyone, it expressed via my lungs. I developed a persistant cough and shortness of breath that lasted for weeks. I thought I had pneumonia as I could not even trudge slowly up a flight of stairs (taking about 10 minutes to climb 12 steps), without collapsing to my knees, gasping for air at the top of the steps. I was basically confined to my bed. Eventually I figured out (not my family practioner, mind you, it was me who thought of it) that it was the methotrexate. I called my Rheumatologist who told me to stop the MTX and in 24 hrs I was back to normal. Further testing by a pulmonologist revealed no permanent respiratory damage. First I was on ibuprofen, then when that quit working I switched to Sulidac. Then we added Plaquinel and then Methotrexate. This all occcured over about a 5 or 6 yr period of time. I'd guess I was on the MTX anywhere from 6 months to a year.
