The Cortisol Production Pathway

In order to understand how Cushing’s syndrome arises, it is necessary

to briefly examine the cortisol pathway. First, the hypothalamus

secretes corticotropin releasing hormone (CRH). CRH stimulates the

pituitary gland to secrete ACTH, which causes the adrenal glands to

release cortisol into the bloodstream. A biologic feedback loop exists

in which the hypothalamus and pituitary release less CRH and ACTH when

cortisol reaches adequate levels in the bloodstream. However, there

are a number of situations that may upset the cortisol feedback loop,

leading to excess cortisol secretion, and hence, Cushing’s syndrome.

Hello, . The most common link between Cushing's syndrome and RA

is that people who are on prednisone for RA can develop Cushing's or

Cushing's-like symptoms.

But there are some interesting case reports in the literature like this one:

Thanks for the info. I should have mentioned the reason I asked. I was diagnosed

with RA about 8 years ago and a few years later my daughter developed Cushings.

I was concerned that there was a hereditary issue, but according to the research

I can find that's not the case. She did have asthma as a child and used

prednisone for it.
