I've obviously been able to document it, because I've never had any problems with it, nor had any complaints from him. However, he is not dictating pachylaryngea/ia and I can't document either word anywhere. Any suggestions? He dictates: On examination today, she has pachylarngea, suggestive of reflux... TIA!
