Rich La Belle wrote: We have been asked to forward the following notice: APD is part of the Florida Disability and Health Project, which is afive-year, federally-funded initiative which seeks to improve the qualityand availability of health care services for Floridians with disabilities.This project is coordinated by the University of Florida and the FloridaDepartment of Health.As part of this project, a brief on-line survey (described below) wasdeveloped to gather information on the importance of various health-relatedissues (as reported by people with disabilities and others involved in theprovision of services and supports to

such individuals). APD has beenasked to distribute this survey to all of our own staff as well as ourstakeholder groups.The survey may be accessed by clicking on the following link:http://FODH.HealthIssuesSurvey1.sgizmo.com We would appreciate it verymuch if you could set aside a few minutes to take it. Even though theinstructions at the beginning of the survey indicate that it will takeapproximately 15 minutes, you can be done in 5-8 minutes, depending on theoptional comments you wish to enter. Your thoughts are very important toAPD and to the success of the project. There are no restrictions on whomay take this survey, so please share the web address with individuals withdevelopmental disabilities, families, waiver support coordinators,providers, and anyone else who you think would be interested.Note: This survey is fully compliant with Section 508
