If you are in Palm Beach County, it's in Delray Beach off Congress south of Atlantic Avenue. Make sure you call for an appointment though, as if you show up, they will send you home without one.

Vaccine exemption form (where and how do I get one?)

Hi,

Iam interested in obtaining a medical/religious vaccine exemption form

for my twins girls who are now 3 and would like to do this before they

turn 4 as they are due for more vaccinations then and would not be

able to enter the VPK program without them. If anyone can direct me on

where and how to get that I'd really appreciate it!!!
