I have heard of it, but don't know anyone who has had to have it done twice. You will be done with the casts in no time and it will be behind you. Guest guest Posted May 15, 2000 Report Share Posted May 15, 2000 Yes, I have heard that some doctors have suggested it and although I don't know if Dr. Ponseti ever had it happen, I did ask Dr. Colburn about this. (Dr. Colburn is a Ponseti method doctor here at our site) Dr. Colburn said he could see how it could happen. If I can relate what he said, I believe he said that during the tenotomy, it is possible to nick the achilles giving the appearance/feel of a 'complete severing' when in fact, it has only partially been cut. He seemed to think that,especially with a doctor NEW to the Ponseti method, that inexperience with performing tenotomies could certainly make it possible for the tenotomy to be incomplete. I don't know who exactly did the tenotomy in Iowa, but I suppose, since it is a teaching hospital, that a resident assisted in the proceedure and maybe it was not a complete tenotomy. Of course that is a LARGE assumption, and I don't want to imply that that is what happened. Only that it is one of many possibilities. There are probably many reasons why another tenotomy may be needed. Of course, I really don't know what the reason is in 's case, it may NOT be that the tenotomy was only partially done. I suppose it could be from a very tight achilles or something, but I do think having to redo a tenotomy is not all that common. But if her foot is not dorsiflexing well, it seems that this is the normal course of action that they would take and certainly least invasive. That had to be some depressing news regardless of why/how. Hopefully the 3 weeks will pass by quickly and you will be back to the regular routine of happy bathtime soon. At once we were told that Ivy would have to have another tenonomy and were fully prepared for that. Ivy had a tenonomy done at three months by an " non Ponseti " Dr. We switched to Dr. Ponseti when Ivy was five months old. She had on 18 casts all together. (Only three of those done by Dr. Ponseti) I can also fully empathize with the disappointment and discouragement that you feel about Lizzy's foot. We too have gone through a " relapse " ; You think that you are done with the casting but things are just not " right " Some positive points about this though are, one that Lizzy is still able to be corrected without surgery. Secondly, three weeks of cast is better than three months which would be close to what she would have to be in if she was to have a surgery. Thirdly, Lizzy will not remember this... Mom always will but she won't. I truly am sorry for you and Lizzy. However when the cast come off after this next three weeks it will be a time to rejoice over those free feet! Guest guest Posted May 16, 2000 Report Share Posted May 16, 2000 , Christian did not need the tenotomy twice, but initially we thought he was not going to need the tenotomy at all. We were working to stretch the achilles through casting but because the achilles was too tight his midfoot began to over-correct (the beginnings of a rocker bottom). So not only did we need the tenotomy but we had to allow the over-correction to release before proceeding with the tenotomy. This meant extra trips to land and the huge disappointment of thinking we were almost done when in fact we were far from finished. I understand what you are feeling. It is very frustrating, at one point I even went through some feelings of whether I made the right choice of treatment, was this going to be worth it...etc.. I had a few good cries and tried to find something positive. (In this case we made arrangements for Ed to accompany me when Christian had the tenotomy, with just the three of us minus the two older sisters it was like a big " date " . We don't get out much!) It was a set back, but we got through it and we are very pleased with the results. Have faith, ask questions if you have them, and try to keep in mind that in the big picture an extra three weeks isn't so bad. Although when you're facing it does seem like an eternity! Dr. H also practices in a teaching hospital and it is unnerving to think that a resident could be performing their first tenotomy on your child. Dr. H allowed residents to watch, and help hold, but in our case he did all the manipulations and castings and the tenotomy himself. Every Dr. has to do their first casting, their first tenotomy etc.. I also worked in a teaching hospital as a secretary in an ICU, and know that residents are well supervised by senior residents and attending physicians. This is a difficult issue though. Anyway, , good luck, and you'll be giving those real baths again before you know it! Dr Ponseti told me that when I brought her down 2 weeks ago with concerns that her feet were a little cramped in her casts, they removed the casts and put her in the DBB. Dr P said they should have put another cast on. I don't know, kinda upsets me that they only sent a resident to check on her and not Dr weinstein. Sure hope the next 3 weeks fly by! Rose was casted until she was 7 months old! Many kids who end up with regular surgery can be casted for longer than that. It is very frustrating, though, when things don't go the way they're supposed to. I hope this 3 weeks flies by for you and Lizzy. Take care.
