Guest guest Posted February 7, 2011 Report Share Posted February 7, 2011 , even though your PSA scores aren't that high, the hardness on one side of the prostate is a sure sign that a biopsy is a good idea -- that's what my GP and three urologists told me before I had mine. My PSA was 4.95, so that alone was good enough to strongly suggest a biopsy, even without the slight firmness they all detected. Regarding the biopsy spreading the cancer, both urologists I asked said there was no evidence suggesting that it did so. I believe I saw an article (on the Prostate Cancer Research Institute website, I think) saying the same thing. BTW, my biopsy showed cancer, with 7 of 12 samples being cancerous for a Gleason score of 7 (3+4). I chose robotic surgery and the pathology report after the surgery showed cancer in both lobes of the prostate, but no cancer outside in the lymph nodes. So far a year later my PSA remains undetectable -- the bests outcome you can have. I wish you well with the biopsy. in the meantime, read everything you can so that if the biopsy is positive you will be on your way to making a decision about what treatment is best for you. Here's hoping the biospy is negative. My PSA a year ago was 2.19 this year it us to 2.79 then just three months later it is now up to 3.01. Not sure if me being on a new diet might have changed the last one or not. I don't know what my staging unit is though or what that is even about. Sorry for not knowing these things. They did find a hardness on one side the specialist said and that is the reason for the biopsy. For most parts there is not much one can do without having the biopsy I suppose. So that is why it is booked along with help here from the group. Just a touch scared I guess. Hard for me even saying that as I was raised not to show my emotions sign of weakness. Did me well for a good number of years I have to say that and still does to some extent but now this came along has me a bit worried. Well rambled on here enough, get back to my shop work keeps my mind on the straight and narrow. Cold here today down to minus 10 and snowing. Based on what you said about your digital exam I would estimate, for the sake of calculating a "what if" Partin Table example that your Staging may be T2b (a tumor involving less than or equal to one lobe), your PSA is 3.01 ng/ml and further assuming your biopsy comes back positive, your Gleason would most likely be 3+3. For that set of assumptions the Partin Tables say the following: Organ confined: 74 (68-80) Extraprostatic extension: 23 (18-29) Seminal Vesicle Invasion: 2 (1-5) Lymph Node Invasion: 0 (0-1) If this were the case then your probability of extraprostatic extension, seminal vesicle invasion and/or lymph node invasion would be 20% to 32% (average 26%); thereby making the added concern of "spreading" your PCa as the result of your biopsy a relatively minor issue since you would probably want to choose a treatment any ways that would not only try to get ride of PCa inside your prostate but also any PCa that might happen to exist just outside your prostate. About 2/3 of all men (according to the 0.2 ng/ml average 10 year "disease free" measure) who are treated at the better centers with seeds only don't appear to be effected by their biopsies so hopefully that is not a major issue. If you, or anyone else, wants to see a recent comparison of the "best of the best" average 10 year PCa treatment results, based on 60 applicable medical journal references, just send me your email address and I will send you a summary chart and/or the entire report. FYI, the Partin Tables were developed at s Hopkins by Dr. Partin and others by comparing the pre-RP PSA, Gleason and Staging numbers with post-treatment analysis of the removed prostate and surrounding tissue. Then after hundreds of RP's they just assembled the results into the Partin Tables so all PCa patients could get an idea of the likelihood of them also having PCa outside their prostate before they selected a treatment..
