I recommend to all my clients, especially if they have medical issues to name someone as their health care surrogate, health power of attorney, etc. Every state has different laws and different names for these documents. So, I can not give any general advice, other than they are a good idea to have. Frequently, hospitals and hospice centers have them for no charge and have notaries right there to execute the documents properly. They are generally not complicated, but you need to make sure that you have the right number of witnesses and notary, if your state requires them. Otherwise the document may be void.
