However, other factors could be at work. For example .. 1. May have taken a prophylactic AA followed by LHRH-a to which he is responding with reduced PSA. [ , what are your androgens doing right now? Have you a healthy level of testosterone, DHT, estrogen, SHBG ? ] 2. The cancer may have shifted gear and just be producing less PSA (but the same amount of tumor). [ Unlikely but you can't rule anything out in this game.] 3. May have seen the light, started to exercise regularly, reduced BMI < 25 and started to eat sensibly (fresh food, not junk food). 4. Finally, removed himself from a source of xenobiotic that was causing the tumor in the first place. [ Remember Doug Adam on your YANA list.] Generally, I'd agree to think twice about surgery, if there have been no other interventions (1) and all his 'bloods' are in normal range; plus, if freePSA% & PSA density are going in the right direction. If there are no symptoms; a pelvic scan shows no sign of worsening tumor: generally not (2). I have copied this note to and PCS.
