New Rheumy appointment next week - suggested questions/medical history info to take in?

Hi,

I just switched to Kaiser (center 3 miles from the house and I'm use to

HMO as

I was in the Air Force for 13 years). I have Fibro, RA, and sleep

aphea. I go to the new Rheumy for my first appointment next week -

suggested questions/medical history info to take in?

I

usually take in a 1 page summary of medical history and questions to

give them

the bullet and the things I will forget if not on paper. I would like

some suggestions as to the questions to have on the bottom of the

page. I

thought:

47-year-old white female.

History of three Laparoscopic

Surgeries (one laser) for endometriosis

(stage 4). I was given drug to control

without the recommended calcium supplement and 30% of calcium lost from

bones. I had a complete hysterectomy

in

November 1990. Hormone drugs for 2 weeks, gain 15 pounds; stopped.

I had my

gall bladder removed in May 1989 for

non-cancerous tumor.

2 drug

induced ulcers (1988 on experimental drug

and 2003 on diclofenac 50mg/ misoprostol 200 mcg tabs).

Just

before I got out of the AF in 1992, I was

diagnosed with a “sleep disorder that causes swelling in your hands” – Fibromyalgia? In 1994,

I was

diagnosed with sleep apnea, restless

legs, and

Fibromyalgia, and RA. I

have a CPAP machine.

1.

I need an

anti-inflammatory that my

stomach will be safe taking.

2.

I have been having a lot

of

coughing and have been bring up clumps of crud for the last 3 weeks.

3.

My

allergies have been bad for the last 3 weeks.

4.

I also am having problems with waking up several

times

during the night to pee and not being able to get to the bathroom in

time. I drink 10 – 14 glasses of water or herb

tea

a day, but I stop at 4 – 5 pm.

5.

I

have been having bad cramping caused by my abdominal scar tissue (5

surgeries

in 5 years). I double over in sudden

waves of pain.

The doctors love having this as it is faster.

Be careful with drinking so much herbal

tea, since it’s medicine.

It could kill your kidneys or cause

reactions with you other meds

Hi Lynette,

Your infomration looks really good and very valuable for a doc. I

like the way you listed your history and then your specific issues

at this time. The only things I suggest adding are a list of any

meds you are currently taking (and supplements too)and the doses, a

list of any medications that you can't take, and a list of anything

you're allergic too.

I hope the visit goes well!

Thanks to everyone who answered. I knew I had frogotten something - a

list of medicine.

, I'm in MD. So, I wont be going to the doctor you were

discussing.

Yellow, I have 4 - 5 cups of the following Earl Grey tea, Mint tea,

cranberry herb, apple herb, or Decafinated green tea. Most of what I

drink is filtered ice water.

Oh good, Lynette. You had me worried for a

minute.

Those are not particularly potent.

Medicinal herbs have to be handled very

carefully and too much is tough on the system.

No problem, thanks for caring. I'm pretty good with herbal medicine

and sometimes use it to supplement the doctors's stuff. I check first.
