I get sick everytime I eat at Burger Queen's (it's been a long time) and Jack in the Box. I'm not so sure about chains. McD's is not so bad because you can get the yogurt fruit walnut salad and sub your own yogurt with the apples and grapes. Let's put it this way: I'll trust a Waffle House steak and egg meal (they're willing to clean the grill for me) with a bit of margarine to cook the eggs in before I'll trust a fast food burger. And I'm unlikely to trust Waffle House unless I really, really get caught.
