Guest guest Posted August 9, 2008 Report Share Posted August 9, 2008 Surgical Pathology of Soft Mass Surgical Diagnosis: Soft Tissue, Right Upper Chest, Subpectoral Region Excision: Xanthogranulomatous inflammation with fat necrosis and refractile irregular foreign material consistent with silicone. Migroscopic: Sections are of lobules of adipose tissue. There is marked fat necrosis with coalescence of the fat cells yielding variably sized microcysts demarcatd by fibrous connective tissue with foreign body giant cells and patchy lymphocystic infiltrate. There is no birefringent material evident under polarized light. Within some of the microcysts, however, there are refractile structures consistent with silicone. Gross: Received in formalin labeled " right upper chest subpectoral mass: is a 7.0 x 4.0 x 1.5 cm roughly ovoid rubbery mass. Attached to the external surfaces of the mass is a small amount of soft tan-yellow adipose tissue. On sectioning the mass has a smooth tan partially cystic cut surface. The cystic areas range from 0.1 to 0.5 cm in greatest dimentions and are filled with clear sebaceous yellow material which is highly suggestive of silicone material. Representative sections are submitted labeled A through C. So, what was thought to maybe be alittle fluid in the right shoulder was that nasty cystic ovid rubbery mass with fat necrosis.......( tissue death ) The surgery was scheduled for 3 hours, and it took her 6 hours to remove it.
