These forms are the official get out of vaccines and the TB test forms for school in CA. They worked at my son's school.

Hello... I plan on exempting my 4 yr old son and new born frOm any further vaccine shots. My 4 yr old regressed into autism after MMR and I dont plan on vaccinating him any more with these booster Shia and I definitely don't plan on any needles coming close to my little newborn. I'd appreciate if anyboy can help guide me on how to exempt gem from vaccines. Thanks!
