You shouldn't have to do anything but flip the blue card (should be in with your k registration packet) over and sign the waiver. You'll need to make sure you put 2 signatures on there, one for the vaccines and one for the TB test. You can exempt pretty much everything - physicals, dental, eye exam, etc but they all have different forms. The blue card is the vaccine card and the waiver is on the back, if you don't have it, just ask for it.

Hello... I plan on exempting my 4 yr old son and new born frOm any further vaccine shots. My 4 yr old regressed into autism after MMR and I dont plan on vaccinating him any more with these booster Shia and I definitely don't plan on any needles coming close to my little newborn. I'd appreciate if anyboy can help guide me on how to exempt gem from vaccines. Thanks!
